Care2x

Care2x is a free, open-source hospital information system (HIS) for hospitals, health centres and teaching institutions that want one web-based system for admissions, outpatient visits, wards, laboratory and pharmacy. It runs on your own PHP server and staff use it in a browser. If you searched for a Care2x download to run a dental practice, read the limitations first: Care2x has no dental charting module.

Be realistic about its maintenance status. The last packaged stable release on the project’s SourceForge page is 2.6.27 from January 2012, followed by a 2.7 alpha in March 2015. Since then a community GitHub repository (care2x/care2x) has carried the code and describes itself as an effort to restart a project that had stagnated. It received commits in December 2025 working on PHP 8 compatibility, but it has no tagged releases.

How dental teams use it

Care2x does not chart teeth, plan treatment or open dental X-rays. Where it appears in dentistry, it is as the hospital-wide system that a dental department sits inside.

Running a dental outpatient clinic inside a hospital

1. Ask the hospital’s Care2x administrator to set up the dental clinic as its own department, so visits and staff are grouped correctly.
2. Register the patient once at admission; the same record is then available to the dental clinic, the lab and the pharmacy.
3. Record the visit and clinical notes as free text or in the forms available to your department.
4. Keep tooth charts and radiographs in dedicated dental software and note the reference in Care2x.

Trying it before you commit

1. Read the README in the GitHub repository. It links to a community demo of the latest code running on an empty database.
2. The maintainer warns that the demo is rebuilt from GitHub and may be broken at any time, so treat it as a preview only.
3. If it looks promising, install your own copy on a test server. Never enter real patient data into a public demo.

Teaching hospital information systems

Because the code is open and the modules map to real hospital departments, Care2x works well in health-informatics courses. Students can install it, follow a patient from admission through lab and pharmacy, and read the PHP code behind each step.

Install and first run

Care2x is a server application. Nothing is installed on the computers at the front desk or in the surgery; they only need a browser.

1. Prepare a server with Apache, PHP and MySQL (or PostgreSQL). Linux is the usual choice.
2. Take the current code from the care2x/care2x repository on GitHub, as its README recommends. The 2.6.27 and 2.7-alpha packages on SourceForge were written for PHP versions that are long out of support.
3. Unzip the archive under your web server’s document root.
4. Make the cache, installer and uploads directories writable by the web server. If cache does not exist, create it.
5. Open the site in a browser and follow the installer.
6. After installation, restrict access to the installer directory and serve the site over HTTPS.

Windows: there is no current Windows installer. The last Windows setup files on SourceForge belong to the 2.5 release candidates from 2007. To test on Windows you need a local Apache, PHP and MySQL stack.

Common gotchas: PHP 8 support is still being worked on in the GitHub code, so expect warnings or broken pages in some modules. Much of the documentation on SourceForge dates from 2007 or earlier.

Limitations

• No dental charting, odontogram, periodontal chart, treatment planning or imaging integration.
• No stable release since 2012. The active GitHub branch has no versioned releases or changelog.
• Patient data would run on code with a small volunteer maintainer base; plan your own security reviews, backups and updates.
• No commercial support. Help comes from the community and from reading the code.
• The interface looks dated next to current hospital systems.
• The project does not claim any regulatory certification.

Care2x vs alternatives

For a hospital-wide open-source system under more active development, look at Bahmni (built on OpenMRS) or OpenEMR, a PHP-based EHR with regular releases and a large community. GNU Health is a hospital and health information system with a dentistry package, which Care2x lacks. If what you actually need is software for a dental practice, a dedicated tool such as OpenMolar fits better than any hospital system.

Is Care2x free?

Yes. Care2x is free and open source under the GPL, and there is no paid edition.

Where does the Care2x download come from?
